Terms and Conditions

The following pertains to Facial Treatment Services:

• I understand the procedure and accept the risks. All of my questions have been answered to my satisfaction and I consent to the terms of this agreement. I do not hold the esthetician, Mea Thompson, responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed.
• I hereby consent to and authorize, Mea Thompson, owner of Blink Beauty Boutique, to perform my facial treatment procedure.
• I have voluntarily chosen to undergo this treatment/procedure after the nature and purpose of this treatment has been explained to me, along with the risks and hazards involved.
• I have read and understand the post-treatment home care instructions. I understand how important it is to follow all instructions given to me for post-treatment care. In the event that I have additional questions or concerns regarding my treatment or suggested home product/post-treatment care, I will consult the esthetician immediately.
• Although it is impossible to list every potential risk and complication, I have been informed of possible benefits, risks, and complications. I also recognize there are no guaranteed results, and that independent results are dependent upon age, skin condition, and lifestyle, and that there is the possibility I may require further treatments of the treated areas to obtain the expected results at an additional cost.

I have also, to the best of my knowledge, given an accurate account of my medical history, including all known allergies or prescription drugs or products I am currently ingesting or using topically. I have read and fully understand this agreement and all the information detailed above. 

The following pertains to Lash Lift/Tint/Brow Lamination aftercare and maintenance instructions:

• No water can come in contact with the treatment area for 24 hours after the application.
• No makeup such as mascara, eyeliner, or brow pencil on the treatment area for the first 24 hours.
• No skincare products or oil-containing products on or around lashes/brows for the first 24 hours.
• No waterproof mascara on your lifted/tinted lashes.
• If redness or irritation worsens over the course of 24 hours please contact your doctor about a potential reaction to the product.
• Do not have any type of permanent makeup or semi-permanent makeup done while your hair is still lifted, as your artist must be able to see the natural direction of the hair.
• No pulling or rubbing of the lashes/brows.

By booking a Lash Lift/Tint/Brow Lamination service you are understanding and agreeing to the care instructions provided by the technician for the use and care of my permed and/or tinted eyelashes and/or eyebrows. This agreement will remain in effect for this procedure and all future follow-ups conducted by the certified eyelash/brow technician. I understand my lash/brow technician will take every precaution to minimize or eliminate negative reactions as much as possible. I will hold him/her and his/her staff harmless and nameless from any liability that may result from this treatment. I certify that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the lash/brow technician, Mea Thompson, responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed today. I have read and fully understand all information in this agreement. I am over 18 years of age and consent to the agreement and to the eyelash lift/tint/brow lamination procedure.

The following pertains to Lash Extension Services:

• I understand that this is a semi-permanent procedure, as my natural lashes will continue to grow and fall out normally, making touch-up or “fill” appointments necessary to maintain the original look achieved by replacing the lashes that have fallen out.
• I understand that a full set of lash extensions can make the appearance of my own lashes about 30-50% thicker, and make my lashes appear 20-50% longer.
• I understand that lash extension services have some inherent risk of irritation to the orbital eye area, including the eye itself, and could result in stinging and burning, blurry vision and potential blindness should the adhesive enter the eye or should an allergic reaction occur.
• I understand that some irritation, itching or burning may occur on the skin if the bonding agent comes into contact with it.
• I understand that if the bonding agent comes into contact with my eye, my eye will be flushed with water and I will be assisted in seeking medical attention immediately.
• I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially envisioned.
• I understand that it is imperative that I disclose all of the information requested in the Client Intake Form.
• I have cited all conditions and circumstances regarding my health history, medications being taken, and any past reactions to products or medications.
• I understand that additional conditions could occur or be discovered during the procedure which could affect my ability to tolerate the procedure.
• I understand and consent to having my eyes closed throughout the procedure.
• I understand that if I have any concerns, I will address them with my lash extension specialist.

I am informing the certified eyelash extension professional of the following conditions:

• Current use of contact lenses which I may be asked to remove during the procedure.
• Current use of eye drops of any kind, prescription or over-the-counter.
• Current allergies or sensitivities.
• History of recurrent eye or tear duct infections.
• History of dry eyes.
• Other medical conditions that would prohibit or compromise placement and retention of eyelash extensions.

I agree to the following eyelash extension follow-up and maintenance instructions:

• No waterproof mascara.
• No oil-based products around the eye area.
• No tinting or perming of eyelash extensions.
• No pulling or rubbing of the eyelash extensions.
• Should any kind of eye drops be necessary extra care should be taken to prevent moisture from coming into contact with the eyelash extensions.

This agreement will remain in effect for this procedure and all future follow-ups conducted by the certified eyelash extension professional. I understand my lash extension specialist will take every precaution to minimize or eliminate negative reactions as much as possible. I will hold him/her and his/her staff harmless and nameless from any liability that may result from this treatment. I certify that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the lash extension specialist, Mea Thompson, responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed today. I have read and fully understand all information in this agreement. I am over 18 years of age and consent to the agreement and to the eyelash extension application procedure.

The following pertains to Waxing Services:

• I do not have any open skin lesions or active herpes outbreak (cold sore or genital).
• I am aware of side effects including, but not limited to: allergic reaction, irritation, redness, burning, swelling, soreness, bruises or bumps.
• I am aware certain medications and over-the-counter products can increase the risk of injury when combined with skin care services. I am not using any medications that may cause such injury/reaction. I will advise my esthetician if this changes.
• I have been off of Accutane for at least 12 months and I am not using Retin-A, any products containing alpha hydroxyl, or doing any other skin thinning treatments.
• I have not used a scrub, microdermabrasion, glycolic peels, or other peels, exfoliated or tanned in the last 72 hours.
• I agree to adhere to all safety post care including: no peels, tanning or wet room services; no swimming/spas/hot tubs for 72 hours after waxing; and all home skin care protocols as recommended by my service provider.
• For Brazilian and/or bikini waxing, I will notify my service provider if I am on my menstrual cycle.
• I am over 18 years of age or I have parental consent co-signed below.
• I understand that my esthetician has the right to refuse services for all waxing if proper hygiene is not followed.

I have also, to the best of my knowledge, given an accurate account of my medical history, including all known allergies or prescription drugs or products I am currently ingesting or using topically. I have read and fully understand this agreement and all the information detailed above. 

By booking a beauty serive you are understanding and agreeing to the following photo and video release agreement. I, hereby grant and authorize Mea Thompson, owner of Blink Beauty Boutique, the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures, videos and /or audio taken of me to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, press kits, websites, social media sites and other print and digital communications, without payment or any other consideration. 

This authorization shall continue indefinitely and extends to all languages, media, formats and markets now known or later discovered. I waive any rights to royalties or other compensation arising or related to the use of the photograph or recording. I understand and agree that these materials shall become the property of and will not be returned. I hereby hold harmless and release from all liability, petitions, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
